We're happy to schedule an appointment for you anytime. Here are a few things to keep in mind when schduling with Dawkins Dermatology.
We see patients during regular office hours by scheduled appointment only. You can download and fill in the Patient Information Record before your first visit. Please arrive 15 minutes prior to your appointment time to complete your check-in.
Minors must be accompanied by a parent or legal guardian for the initial new patient office visits, or they will be asked to reschedule their appointment. If the patient is 16 or 17 year old, they may be seen for follow up visits.
When you call to make an appointment, please ask for the appointment desk. The appointment clerk will make every effort to keep your holding time to a minimum. If you are calling long distance, please tell us so we may expedite your call.
If your insurance plan requires an authorization form your Primary Care Physician (PCP) we will need to have a current copy of your authorization in our office before we can schedule your appointment. Your PCP may fax an authorization to us at 405-470-7093. Please allow 24 hours for us to receive your authorization.
If you want another member of your family to be seen at the time of your appointment, arrangements should be made ahead of time. We may have to reschedule your appointment so you may both be seen at time when two consecutive appointments are available. As in any medical practice, we periodically treat patients with emergent medial problems requiring immediate evaluation and treatment. Due to these and other unforeseen problems, our doctors may run behind schedule, causing delays in your scheduled appointment. We assure you that all efforts will be made to see you as close as possible to your scheduled appointment time. We apologize for any inconvenience you might face.
We schedule appointments up to one year in advance. As a courtesy we try to contact you by telephone the day before your appointment to remind you.
If you need to cancel an appointment, please let us know 24 to 48 hours in advance, so another patient may be seen in your place.
